The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Agency, either directly or through coordination of care with other spiritual counselors.

Responsibilities

  • Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
  • Facilitates development of the interdisciplinary team plan of care to meet identified spiritual needs.
  • Provides direct spiritual care to patients and families/caregivers.
  • Serves as liaison and support to community chaplains, clergy, and spiritual counselors.
  • Documents and maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
  • Documents direct services and ongoing communication with community chaplains, clergy, and spiritual counselors.
  • Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
  • Provides consultation, education, and support to the interdisciplinary group on spiritual care topics as needed.
  • Engages community chaplains, clergy, spiritual counselors, and spiritual care volunteers appropriate to meet patient and family/caregiver needs by developing and maintaining a network of community contacts.
  • Provides for funeral or memorial services for patients as requested.
  • Plans and facilitates periodic memorial services to meet the needs of families, caregivers, hospice personnel, volunteers, and general community.
  • Assists in supervision of spiritual care volunteers when assigned to patients/families/caregivers.

  • Proven experience leading individuals on their personal spiritual journey while not imposing a specific set of personal values
  • Knowledge of traditions and customs related to a variety of different belief structures and ability to get in contact with appropriate resources to meet the needs of the patient
  • A team player with excellent communication and interpersonal skills

  • Outstanding organizational and multi-tasking skills
  • Patient with excellent problem-solving
  • Ability to accurately document services provided with a variety of electronic devices and software.

Requirements

  • Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred.
  • Experience working with death and dying, individuals/family/caregiver preferred.

Pay: $45,000.00 - $55,000.00 per year
